What is Sclerotherapy?
Sclerotherapy is a technique that has been used for many years to treat varicose and spider veins. It involves the injection of a medication (a sclerosant) directly into the veins. The medication causes the lining of the vein to swell, eventually sealing off the vein, and preventing high pressure from getting in the vein. The vein hardens and is absorbed by the body over the course of eight to ten weeks, much like a bruise. The strength of the medication needed is adjusted depending on the size of the vein. At Midwest Vein Center, we use Sotradecol® and Asclera®, the only two sclerosants approved by the FDA as safe and effective for the treatment of spider or varicose veins by sclerotherapy.

How do I know if I need Sclerotherapy?
Prior to treatment, a complete medical history is taken and you will receive a thorough examination in order to determine, among other things, how long the problem has existed, whether there are ongoing causative factors, and if you are experiencing any associated symptoms. A diagnostic ultrasound is often needed to determine if there are underlying causes that need to be treated before beginning Sclerotherapy. If an ultrasound exam shows venous insufficiency in the underlying veins, an Endovenous Laser Procedure may be needed first. Sclerotherapy of symptomatic varicose veins is often considered medically necessary and may be billable to insurance, while the treatment of spider veins with Sclerotherapy is always considered to be a cosmetic procedure and therefore, an out of pocket expense.
Prior to Sclerotherapy
Pre-procedure instructions for Sclerotherapy may include the elimination of certain medications or vitamins which are known to cause excessive bruising or bleeding. During the procedure, you will rest on a comfortable examination table and the skin over the veins is cleansed with alcohol. No anesthetic is necessary! A small amount of sclerosing (hardening) medication is administered directly into the vein using a small syringe and a very tiny needle. The vein immediately disappears as the blood is displaced by the medicine, and then returns in about 10-15 seconds. The vein becomes irritated, red and swollen, prohibiting the reentrance of blood.
The irritated vein will harden (sclerose) and then fade away over a few weeks to a few months. Because the needle used for the injection is very small, and the strength of the medicine used is the lowest dilution needed to be effective, there is minimal discomfort during the procedure. Multiple treatments sessions will generally be needed to achieve desired results.
After the treatment, a comfortable compression stocking is applied and should be worn for a period of 5-7 days, and in some cases longer, to obtain desired results. Strenuous exercise, stationary standing, and a hot tub or sauna should be avoided for 48 hours, but walking and normal daily activities are encouraged. Most patients do not experience discomfort after the procedure, but it is safe to take an over the counter pain reliever if needed.
