What is Endovenous Laser Treatment?

Endovenous laser treatment (EVLT) will often improve the cosmetic appearance of varicose veins. However, its main purpose is to treat advanced cases of venous insufficiency characterized by pain, achiness, swelling, itching, skin changes, or ulcers. This specialized laser treatment has replaced traditional vein stripping and works by closing down and sealing off unhealthy veins below the skin surface. Healthy veins then take over, improving the circulation back to the heart. The pain and swelling caused by varicose veins is eliminated and the skin takes on a healthier look.

How does Endovenous Laser Treatment Work?

EVLT has replaced traditional Vein Stripping by closing down and sealing off unhealthy veins below the skin surface with the use of a laser. Healthy veins then take over, improving the circulation back to the heart. This procedure is performed in our offices and requires no hospitalization or time away from work. It is a very safe, precise and effective procedure that has been performed thousands of times by each of the doctors at Midwest Vein Center.

Our Process

Only local anesthetic is needed so there are no diet or medication restrictions with EVLT. You are awake during the procedure and rest comfortably on an examination table. The leg being treated is cleansed with a disinfectant soap and sterile drapes are put in place. The local anesthetic is administered to the entire area being treated. An ultrasound is used to guide the doctor in positioning the thin laser fiber into the vein through a micro incision. Once the doctor confirms placement of the fiber by ultrasound, the laser fiber is connected to the laser machine which heats the fiber tip. As the laser fiber is withdrawn, the heat from the laser closes the vein.

Immediately following the procedure, a small steri-strip is applied to the micro- incision. No stitches are necessary! A gauze bandage is placed over the steri-strip and a compression stocking is applied to the entire leg for comfort. You will rest on the examination table with your legs elevated for an additional 15 minutes after the procedure. Normal activities may be resumed immediately, and you can drive yourself to and from our clinic. The average length of an EVLT treatment is only 30-60 minutes from start to finish!

After Endovenous Laser Treatment 

The compression stocking provides comfort and may be worn each day until returning to the office for a check up in 5-7 days. Resuming normal daily activities is encouraged. Strenuous exercise may increase the amount of discomfort, therefore it is recommended to cut your exercise routine in half for a few days and to gradually increase as tolerated. Over the counter pain relievers will relieve any discomfort.
A follow up ultrasound exam is necessary 5-7 days following the EVLT procedure to make sure the treated vein is healing as it should. Additional ultrasound exams will also be needed to monitor your progress over time. For some patients, the surface varicose veins shrink in size within a few days to a few weeks, and others may not see a significant change. Additional treatment like Sclerotherapy or Micro-Phlebectomy might be recommended to eliminate persistent surface veins, if desired.

Are there any risks?

Serious complications following EVLT are rare when the procedure is performed by a properly trained physician. Because EVLT is performed under sterile conditions, the risk of infection is very low. Occasionally, nerve irritation resulting in a feeling of numbness near the treated vein may occur. While this usually resolves over time, it is possible for the numbness to be permanent. Although extremely rare, the occurrence of a deep vein clot has been reported in medical literature and is mentioned here only in the spirit of full disclosure.

Will my insurance cover the treatment?

Our insurance specialists will send a letter to your insurance company outlining the doctor’s plan for your treatment. Because we have an experienced staff dedicated to this process, we are able to obtain this information for you and notify you when the process is complete.

We are contracted with all the major insurance carriers (Medicare, BCBS, United Healthcare, Humana, Cigna, Humana). However, IF you are insured with an HMO type of insurance, it is your responsibility to bring a signed referral or authorization form from your Primary Care Physician (PCP) to each appointment.
